Ineligible risks (if applicable):

  • Amusement centers
  • Dedicated martial arts/MMA facilities
  • Facilities running sports leagues without a Sports Accident policy in place

The following services are ineligible if a separate Professional Liability policy is not in place or the services are excluded from the commercial General Liability policy:

  • Tattoo or micropigmentation facilities
  • Spas providing cryotherapy, microdermabrasion, Botox or injections, or medical procedures

Geography:

Nationwide, except LA, MT and VA

Submission requirements:

  • Forms available upon request from Program Administrator
  • ACORD applications (including a complete SOV)
  • Supplemental applications
  • Currently valued loss runs for the past five years
  • Financial statements

  • Commercial Auto, including GKLL for Valet Services
